Transportation Providers Inventory and Survey

The Transportation Providers Inventory not only provides information for the transportation planning process, but also for elderly persons, low-income persons, persons with disabilities, and others in need of transportation.

If you are a transportation provider, please take a moment to review the current Transportation Providers Inventory and email any changes to transportation@incog.org.

If you are not listed in the inventory, please download the 2008 Transportation Providers Survey and follow the steps below to submit your survey. Note that the survey requires Adobe Acrobat Reader.

To submit the form by email to transportation@incog.org

• Complete the survey electronically

• On the Menu bar, click "File"

• Select "Send" and then "Page by Email" (In some versions, you may see a pop-up window that asks what format you would like to send. Select "Send Copy.")

• If you do not have these options, Click "File" and "Save" to save a copy to your computer, and then email as an attachment

To submit the form by FAX or Mail

• On the Menu bar, click "File" and "Print" and complete the survey by hand.

• Send by FAX to 918.579.9519
